Insurance workflow
Claims decisions combine policy terms, documents, assessments, authority and judgement under time pressure. A later reviewer needs more than a payment event.
Definition
Claims decision assurance preserves the point-in-time basis of a coverage, reserve, settlement or decline decision and makes its evidence relationships reviewable.

Independent review
The envelope can link model output, runtime records, policy versions, authority records, claims documents, human rationale, external data and approvals while preserving each source's declared limits.
Insurance example
A settlement decision retains the relevant policy version, loss evidence, adjuster assessment, authority threshold, approval, exception rationale and committed amount.
Evidence map
Claim files commonly span a claims platform, document store, correspondence, vendor reports and approval queues. When a settlement is challenged, the reconstruction burden is the work of finding which facts and authority actually informed the commitment.
Material facts and documents known in the decision context.
Evidence declared to have informed the decision.
Role, mandate, limit and approval state at execution.
Settlement, reserve movement or other organisational consequence.
Later claim outcomes can inform a learning loop, but they should not be projected backward as if they were known at execution. Decision assurance keeps the original state and later outcome distinct.
The VES connection
VES 1.2 can represent claim documents, system events, policy excerpts and human reviews as Evidence Objects linked to one Decision Envelope.
Read VES 1.2 for the published Decision Envelope, Evidence Object and verification model.
Boundary
A preserved claims package does not establish that coverage was legally correct or that every relevant document was captured. It makes the declared basis and gaps visible.
